To request a refill please observe the following:

Unless waived by Dr. Scott, we will need to see you for a brief zoom refill visit to catch up on your progress, any changes in status, and your response to the medicine, in order to process your refill request.

To keep this process as simple as possible, it is now possible for you to self-schedule this visit, using the calendar link here.

Prior to scheduling this visit, OR if visit is waived, please do the following:

  1. Fill out the refill follow up form here. This step begins the refill process by starting the note, and triggers my assistant to pull necessary documentation and track scheduling and payment.

  2. Pay for the refill visit which will be $175. You may pay here including credit card fees of 3%. If you prefer to avoid paying the 3% fee, you may make payments through Zelle, using the ID welcome@sapiencetherapy.com, OR you may mail a check to 25 Bank Row, Suite 3, Greenfield MA 01301. Please note your name, date of service, and service being paid for on your check.

If you are unable to find a time that works for your schedule using the link above, please let us know, and we will work to find you a time.

Note that the ketamine will need to be mailed to you from a compounding pharmacy out of state, via UPS or FedEx, and usually will need a signature. If there is a day of the week, or a stretch of time coming up, that you know there will not be anyone in your home to sign for the package, please let us know right away so we can schedule around this. Please ensure you update your address on the refill request form.

Please note: It usually takes a minimum of a week for the ketamine to get to you unless we pay extra for rush processing. If you feel you need the medicine sooner than that, indicate that on the refill form above, and we will request that for an additional fee from the pharmacy.

Please also note that we will provide tracking information to you as soon as we receive it from the pharmacy. Unless you have requested and paid for rush processing, please allow at least 5 business days before inquiring to ask for this number. If after this time has passed, you find you must inquire about the tracking number, please reach out to front desk staff for this information, at scheduling@sapiencetherapy.com.

Be aware that a prescription for ketamine will, like other controlled medications, be visible to other providers should they check your data on the Prescription Monitoring Program, which is required by law prior to prescribing any controlled substances. This prescription is of course completely legal. Please let us know if you have any questions about this.

If you will be receiving ketamine prescriptions for longer than 6 months, we will need to monitor your liver function enzyme results (LFTs) annually. Please send us your latest LFT lab results if you have not done so within the past one year. If you do not have a recent set, please request that your outpatient provider order this test the next time you have lab work done. If you need a lab slip, we can generate one for you, but it will likely not be covered by insurance so it is generally preferred to go through your established PCP or other outpatient medical provider.

Please also note the blood pressure monitoring requirements and keep a log of your home blood pressures, which you can share with us at your refill visit.
