Sapience Therapy offers ketamine assisted psychotherapy for mental health indications including depression, anxiety and PTSD, generally in-office.
The KAP (ketamine assisted psychotherapy) process begins with a brief screening to rule out medical or psychological contraindications to ketamine therapy, at no charge. If no contraindications are found, a medical intake is scheduled with one of our prescribers and a psychological intake is scheduled with the KAP therapist you will see. During these intake sessions we will go over your medical and psychiatric history, personal experience with psychedelics, current reason for seeking treatment, and discuss the possible effects you might experience during a course of ketamine therapy,and answer any questions you may have.
If, after the intake process, proceeding with ketamine therapy seems indicated and agreed on by both you and the prescriber, then ketamine lozenges will be prescribed and mailed either to you or to our clinic by a compounding pharmacy. If mailed to you, you must bring the medication unopened to your first session.
Future KAP sessions can be conducted with ketamine lozenges or with intramuscular (IM) ketamine, depending on the specifics of your situation and your personal response to the medicine. We recommend that new clients schedule a series of six KAP sessions, spaced weekly or every other week. We have found that six sessions is generally the lowest number of sessions that can produce a full and robust response. Partial response is often seen after the first session, but sometimes not fully appreciated until after two or three sessions. Scheduling the full six session series at the time of engagement is highly recommended to ensure that appointment times will be available to you should you desire to complete the full course of treatment. Should you decide KAP is not the right treatment for you, these sessions can be canceled without penalty up to 48 hours before the appointment.
Prior to a ketamine session we ask that patients do not eat for 4 hours, to reduce the risk of nausea. You will need a ride home after any medicine sessions, and will be instructed not to drive for the rest of the day.
Please note that KAP sessions are three hours in length. Most Sapience therapists offer sessions at 10 am and 1:30 pm. If you suspect you will need alternative session times, please check with us about availability before scheduling your intake appointments.
To ensure patients have adequate support outside of KAP sessions, we typically require patients to have an established relationship with a therapist they are seeing regularly. In the future, we hope to have therapists on staff who can accept clients who do not already have a therapist. We may want to speak with your therapist or psychiatric provider for assessment or collaborative purposes.
Sapience Therapy is proud to be an internship site for the Smith College School of Social Work. At times, we may have a limited number of reduced cost KAP slots available, facilitated by our social work intern, under the supervision of one of our fully licensed clinicians. At this time there is a waiting list for such sessions. Please request further information if this is an option you might be interested in.
We also offer low cost preparation and integration sessions over zoom with an integration coach, which can assist clients in getting a more robust response after their sessions. They can be scheduled at the time you schedule your KAP sessions, or added later.
We have also begun offering small group KAP, which involves a group of 4-8 participants, meeting both in person and on zoom, with a total of four 3-hour KAP sessions (in person) and multiple preparation and integration sessions. Details can be found on our website. The next cohort is being planned and dates will be released soon; if you are interested please let us know and we will keep you updated as dates are finalized!

Appointments missed with less than 48 hours notice will incur a fee of $50.
Unfortunately because ketamine for mental health indications is not covered by most insurance, we do not currently accept any insurance. We can give you an invoice (“superbill”) to submit to your insurance company if desired.
Our practice is supported by Osmind, which offers an online patient portal / app, and a mood monitoring program. If you decide to work with us, you will receive welcome information including intake forms, information forms, and consent forms emailed to you from the Osmind platform. Please take the time to fill out and sign all of these prior to your first appointment.
The office is located at 25 Bank Row St, Suite 3 in Greenfield MA. The parking garage is just around the corner on Olive Street. Come to the third floor using the stairs or elevator, and once there, there will be a hallway to your left. Our office is the last door on that hallway.
